Form W-9 (blank) - Request for Taxpayer Identification Number and Certificate
The W-9 form is used to collect pertinent tax information from a payee before a vendor ID or payment will be processed by UC Merced. If you need to collect tax information from a vendor, please provide the prospective vendor the blank UC Form W-9. If you need to collect tax information from an entity other than a vendor, please provide the prospective payee the official IRS Form W-9, published on the IRS website.
Do not send W-9 forms to the Internal Revenue Service (IRS). Instead, please ask the vendor/payee to send the form via fax to (866) 397-9281 or via email to vck@ucmerced.edu.
